1. A referral from your Primary Care Physician (PCP) or Surgeon.
2. Relevant Doctor's notes (Soap Notes) and or visit summaries, at least the last 3 visits.
3. MRI Paper Reports done within the last 3 years.
The preferred and verified method is for items 1,2 & 3 to be faxed to 321-338-2977 from your Dr's Office / Imaging Center prior to requesting a (pre-admittance evaluation) preliminary appointment. If needed click the Medical Records Release Form button to download, print, fill out and deliver to your doctors office and or Imaging Center. Rivercoast Pain Management is a medical practice owned and managed by a board-certified Anesthesiologist. Our focus is on whole body wellness and the treatment of those suffering with chronic pain. Our mission is to assist you in your journey of educated self care, improved functionality and maximum wellness.
Treatment modalities will include, but are not limited to, medical intervention; nerve, joint and intramuscular blocks; massage; and acupuncture. Any treatment options that we do not provide at Rivercoast will be referred out to health care partners we trust, because your total well-being is our only mission.
Since your mental state is so important to the improvement of your general health and functionality, we will also consult with and refer to psychologists, mental health counselors, or addiction specialists as deemed necessary.

As a part of improving patient health and general well being, a more comprehensive approach to pain management care will be used:
